Set filters and columns, then Run report — the page shows a preview of up to 100 rows. Download CSV exports the full filtered list (same sort order, no row cap). Click column headers to re-sort the preview only. Update lists refreshes city/ZIP dropdowns.

1. Filters

Use the lists to narrow providers. After choosing states, click Update lists to load cities and ZIPs. Ctrl+click (Windows) or ⌘+click (Mac) to pick several options. Leave filters empty to browse a preview slice; use Download CSV for the complete matching set.
